Scheduling an Appointment
Sessions are by appointment only. My usual hours are Monday through Friday, approximately 9 a.m. to 4:45 p.m. On weekends, I am available for emergencies only.
Please phone me at (301) 675-0693 to schedule an appointment. Please include your phone numbers, email addresses, home address, and the patient’s date of birth.
Before the first scheduled session, if possible,
a) Please print, read, sign and bring the Practice Guideline and Consent to Treatment Document to the session. I am glad to address any questions you may have about these guidelines during our session.

b) Log into the patient portal and complete the demographic and insurance portions.
c) If you have had prior psychiatric treatment, such as with a social worker, psychologist, or psychiatrist, please bring contact information. Lastly, if any educational or psychological testing has been done, please bring a copy of the report with you to our meeting.
